SSS Tarlac needs certified documents on Astini News
This is in reference to a letter from Erlinda M. Alfonso regarding the permanent total disability (PTD) of her deceased husband, Virgilio L. Alfonso.
Erlinda said: "My husband, Virgilio L. Alfonso (SSS No. 02-0060193-7) got injured in 2002 and was granted partial disability thereafter. On Jan. 1, 2004, he had a CVA (cerebrovascular accident). In 2006, as the surviving spouse I filed for permanent total disability (PTD) under PD 626, ECC law. Until now the claim is still with the SSS Tarlac branch. A motion for reconsideration filed on March 24, 2011 remains unacted upon until today. The SSS has to understand my situation. I am asking the SSS Tarlac branch to help resolve my claim promptly."
According to SSS Tarlac, Virgilio's EC disability claim was denied because he was no longer employed at the time of the accident. He retired in 2002 and his wife, Erlinda, filed the EC claim only after retirement.
Erlinda made an appeal in March 2011. SSS Tarlac could not forward the case to the Medical Operations Department (MOD) because Erlinda failed to submit the required documents.
On July 18, 2011, SSS Tarlac wrote to Erlinda asking her to comply with the documentary requirements which must be certified by Hacienda Luisita, her husband's employer.
SSS Tarlac needs the documents before it can endorse the case to the MOD for submission to the Employees Compensation Commission.
